OVERVIEW

Despite the introduction of the Biopsychosocial Model to medicine thirty-two years ago, our culture and our health care system continue to operate from an artificial divide between physical health and mental health, including substance abuse and other behavioral health
problems. Primary care practitioners are usually the first health system contact for patients with mental health problems, frequently recommend mental health services, and treat the common problems of anxiety, depression and substance abuse. Mental health practitioners often refer patients to primary care for consideration of pharmacotherapy and for basic primary health care. Research has demonstrated the importance of identifying and treating the mental
health components of chronic medical problems such as diabetes and there is increasing evidence supporting the effectiveness of behavioral therapies for lifestyle modification.
The current divide between medical and mental health or substance abuse practitioners is a barrier to achieving the highest quality care in Vermont and elsewhere across the country. |

Conference Goal
To bring primary care and mental health practitioners
together to help bridge the divide in Vermont; health
policy experts and others will participate. |

Desired Outcomes
Primary care, mental health, and substance abuse practitioners learn more about each others’ roles,
frustrations, and aspirations in providing care for the
people of Vermont. Everyone becomes aware of examples of successful collaborations between primary care and mental health practices, both regionally and nationally.
Participants outline a potential framework for systems
of local collaboration, communication, and referral,
and set up some next steps. |
